“Solutions-Oriented Approaches to Improving Healthcare for Older Adults” Conference at RAND Thursday June 20th.
The “Solutions-Oriented Approaches to Improving Healthcare for Older Adults” conference is the first of a series of events around seniors our Center will host over the coming months.
Focused on using applied research to create interventions that improve healthcare delivery for seniors, presenters from four dynamic Southern California healthcare organizations—Cedars-Sinai Medical Center, Facey Medical Foundation, Kaiser Permanente Southern California, and HealthCare Partners—share their knowledge and experience in addressing the needs and challenges of proactively managing the health of the growing older adult population. The conference has been designed to provide a stimulating interprofessional opportunity to explore practical solutions aimed at enhancing care transitions, improving clinical outcomes, and stimulating the active engagement of at-risk patients. We appreciate the generous support of to the SCAN Health Plan® Community Giving Program and our collaborator, the HealthCare Partners Institute for Applied Research and Education.
