USC/RAND “Solutions-Oriented Approaches to Improving Healthcare for Older Adults” Conference Held June 20th
Nearly 60 medical directors, care managers, researchers, students and health and seniors services professionals gathered at RAND June 20th to explore applied research and practical solutions aimed towards enhancing care transitions, clinical outcomes, and at-risk patient engagement. Presenters from four dynamic Southern California healthcare organizations—Cedars-Sinai, Facey Medical Foundation, HealthCare Partners Medical Group and Kaiser Permanente—shared their knowledge and experience in addressing the needs and challenges of proactively managing the health of our aging population. The Center hosted the conference in collaboration with the HealthCare Partners Institute for Applied Research and Education. It was the first in a series of conferences and workshops related to our SCAN® Improving Healthcare for Older Adults project. Read highlights and access conference materials.
