Center Hosting Population Health Management Workshop at HSC

Join us for a special workshop, POPULATION HEALTH MANAGEMENT: FROM DATA TO DELIVERY on Tuesday, November 19, 2013 from 8:30-11:30 a.m. at the USC Health Sciences Campus in Room 2902 of the new Soto Street II Building, located at 2011 N. Soto Street.


The agenda USCSCAN-Nov19Workshop-Information-Agenda for this practical, interactive workshop will focus on creation and use of chronic disease registries for population health monitoring, and for informing development of innovative health programs to improve patient health and engagement. HealthCare Partners Affiliates Medical Group (HCPAMG), one of the nation’s largest and well-developed integrated delivery systems, will serve as a case study organization. Medical informatics issues and disease registry challenges will be discussed. Descriptive data profiles will be presented for two large registry patient populations–Diabetes and Chronic Obstructive Pulmonary Disease (COPD). Two related interventions–Diabetes Group Appointments and use of interactive voice response (IVR) technology for COPD patient home management–will be highlighted as tested disease management strategies.


Faculty, researchers, and graduate/medical students are welcome. As the workshop is being underwritten through a SCAN Health Plan® Community Benefit grant, there is no registration fee; however, space is limited.
